Why the postcode
The islands have postcodes of their own. With Alphanumeric and Is one of you enter each one, and with "*" as a wildcard whole beginnings.
Imke ships tea from Leer, including to the East Frisian Islands. Her parcel service charges extra for every delivery there. When the delivery address is on Borkum, Juist, Norderney, Baltrum, Langeoog, Spiekeroog or Wangerooge, the plugin adds an island surcharge of €4.90 to the cart, recognised by the postcode.
On the left the rule in the Rule Builder, on the right the item's fields that matter. Everything else stays as it is when you create the item.

Postcodes are only unique within one country. With Shipping address: Country Germany the rule only matches German addresses.
The customer sees what they pay for, and the surcharge applies to every shipping method. The shipping methods' price matrices stay as they are.
What to watch out for: The cart only knows the postcode once the delivery address is set. Before that the surcharge is not there yet. If the customer should read about it on the product page already, add a product note for all products.
With a rule made of Shipping address: Country and Shipping address: Postal code, and an item at a fixed amount. You enter the islands' postcodes in the condition and the surcharge on the item. If the delivery address is on one of them, the island surcharge appears as a line of its own in the cart.

The cart only knows the postcode once there is a delivery address, that is after logging in or when a guest enters it at checkout. Until then the rule does not apply. The delivery country, on the other hand, is known before that.
With a second item whose rule only checks Shipping address: Country Is one of Switzerland, for instance €9.90 as an international shipping surcharge. The same works for any other country, also as a percentage. With a negative amount it becomes a subsidy, as in the shipping subsidy example.
Yes, through a price matrix with a rule in every shipping method. The extra charge is then part of the shipping costs, and you maintain it in every shipping method. With the plugin the island surcharge is a line of its own that applies to all shipping methods.
